We reserve the right to change the terms of this Notice and make the new Notice provisions effective for all PHI we maintain.
If you believe your privacy rights have been violated, you may file a complaint with us by contacting us. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a complaint will not result in retaliation.
