Your Rights

You have the right to receive a copy of your health records, request corrections to your records, request restrictions on how your information is used and disclosed, request confidential communications, and receive a list of certain disclosures we have made of your information. You also have the right to obtain a paper copy of this Notice upon request.

How We May Use and Disclose Your Information

We may use and disclose your protected health information (PHI) for treatment coordination with licensed providers in our network, for operational purposes including quality assessment and improvement activities, and as required by law. We will not use or disclose your PHI for marketing purposes without your written authorization, and we will not sell your PHI.

Our Duties

We are required by law to maintain the privacy and security of your protected health information, provide you with this Notice of our legal duties and privacy practices, notify you following a breach of your unsecured health information, and follow the terms of this Notice. We reserve the right to change the terms of this Notice and make the new Notice provisions effective for all PHI we maintain.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us by contacting us. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a complaint will not result in retaliation.
